Renewing Your Medicaid is Easy

To get started, you’ll need your Medicaid Renewal Form. You’ll also need your Application Reference Number, Renewal Code, or "Integrated Case" Number. You can find all of this information at the top of the document.

The grace period for D1 renewals ended October 31, 2016. If you did not return your D1 renewal form you will have to reapply for Medicaid coverage. If you returned your completed renewal form prior to October 31, 2016, it is being processed and you will hear from us soon. If you have any questions, please call us at (855) 532-5465 TTY DIAL 711.
