IF YOU HAD INDIVIDUAL PRIVATE HEALTH INSURANCE FOR ALL OR PART OF CALENDAR YEAR 2014 THROUGH DC HEALTH LINK
If you had individual private health insurance through DC Health Link during 2014, you will receive a form called Form 1095-A. This form shows how long you had individual (or family) health insurance through DC Health Link in 2014. This form will also show any premium assistance you received. You will need this form for your personal income tax filing. A copy of the Form 1095-A will be mailed at the end of January. You can also download a copy of your Form 1095-A below.
IF YOU HAD DC MEDICAID FOR ALL OR PART OF CALENDAR YEAR 2014
Form 1095-A does not include Medicaid coverage information. You will not receive Form 1095-A from DC Health Link showing your Medicaid coverage.
IF YOU HAD JOB-BASED HEALTH INSURANCE COVERAGE FOR ALL OR PART OF CALENDAR YEAR 2014 THROUGH DC HEALTH LINK
Form 1095-A does not include job-based health insurance coverage information. You will not receive Form 1095-A from DC Health Link showing your job-based coverage.
IF YOU DID NOT HAVE HEALTH COVERAGE IN 2014
If you did not have health coverage for 3 months or more in 2014, you may have to pay a fee to the IRS when you file your 2014 personal federal income tax return. You may qualify for an exemption from the IRS fee.
